Title IX Discrimination and Harassment Reporting Form

This form is designed for anyone (students, faculty, staff, visitors and guests, etc.) to report incidents of harassment and discrimination to 91³Ô¹Ï, including but not limited to:

  • Sexual harassment
  • Sexual misconduct
  • Relationship violence
  • Retaliation
  • Stalking
  • Bullying
  • Hazing

Report an Incident:

Do not use this form to report events presenting an immediate threat. If you require emergency assistance, please contact 91³Ô¹Ï Public Safety at 570-961-7899 or call 911.

91³Ô¹Ï’s Title IX Coordinator (or designee) receives and reviews all complaints and routes them to the appropriate 91³Ô¹Ï administrator for investigation. Questions and concerns can be directed to TitleIX@91³Ô¹Ï.edu

This form can be used by any person making a report on their own behalf who wishes to remain anonymous. It can also be used to anonymously file a report on behalf of another person. 91³Ô¹Ï will investigate every report to the extent possible. However, it is important to understand that if you choose to file an anonymous report, you may limit Title IX’s ability to remedy the problem. 91³Ô¹Ï will make every effort to protect the privacy of all involved, but the information you provide in completing this form is not confidential and may be shared as necessary for Title IX staff to take appropriate action.
